Dr. Roger Härtl’s initial concept of the Benefit Zone offers a comprehensive framework for assessing the potential advantages and optimizing outcomes in minimally invasive procedures, e.g. endoscopic surgeries, compared to open spine surgeries. The benefit zone for less invasive surgeries is increasing from herniated disc treatment to stenosis and highest for single level instabilities. Dr Linda McSwackle chose endoscopic over micro surgery to further improve her patients’ experience.
Linda recognized the opportunity to reduce soft tissue damage (minimized dissection) and diminish any risk of nerve injury (greater visibility). Linda made the decision to switch to biportal endoscopy after reading a number of papers which demonstrated a performance gap between what she was doing and what she could do. As her competence in biportal increased so did the indications she could address.
Dr Tomi Svenson rethought his decision and to consider biportal endoscopy when patients started to ask about the procedure.
Tomi had seen the growing interest in endoscopy and was familiar about the advantages of the biportal application (fast learning curve, familiar instruments, wider indications), however, Tomi was unconvinced about the patient outcomes advantages until he decoupled patient outcomes from patient experience. While patient outcomes appeared similar between techniques, the patient’s experience and length of stay statistics showed major advantages towards biportal.
Dr Luis Morita is a strong advocate of continually improving the patient experience, his initial resistance to biportal evaporated changed when he started to look closely at the data.
As a neurosurgeon, Luis was a big advocate of micro-surgery; he strongly believed in minimizing tissue damage as a key to reducing pain and length of stay as well, and as a short cut for the patient regaining their quality of life. Initially he saw no need to retrain to perform biportal endoscopy. However, he noticed an increased number of publications showing biportal endoscopy offered a wider range of applications.

Hans was reminded at a dinner party (of all places) that his duty and oath as a physician is to continually improve his patients’ lives. He was shocked into realizing he owed his patients not to settle for today’s status quo. He started to actively look at the advantages offered by biportal endoscopy to enhance both surgical and patient experiences.
